To get a prescription for natural hormones, you must first test
your hormones and provide a medical history. This is called a HormoneProfile.
You will receive a simple saliva test kit to do at home. This test measures your key hormones to determine if you need a customized prescription
for natural hormones. When the results come back, you will have a telephone
consultation with a ProactiveHealth medical professional.

insurance covers this program. ProactiveHealth will work with your carrier to get reimbursement. If you have Medicaid, ACCSS Arizona or do not have
health insurance, the price of the test can be billed to your credit card.
How does the program work?
1. Fill out the
Forms - You will fill out a Health Questionnaire with your medical
history and a Registration Form with your insurance information. After filling
out these forms, you include them in the package with the saliva test. This
medical information will be kept in our secure database accessible only to our
licensed health care providers. You can also fax these forms to us if you
wish.
2. You
do an in-home Saliva Test
–To determine if you need a prescription for natural hormones, you must first measure your hormone
levels. ProactiveHealth will send you a saliva test kit containing a pre-paid mailer
for the saliva sample. No need to go to an outside lab. You collect the saliva sample at home
first thing in the morning and mail it back.
3. Telephone
Consultation - You will get an evaluation by a ProactiveHealth licensed
medical professional in a personal phone consultation. Your test results will
be ready about a week after you send it to the lab. Within a day or two
thereafter, your Health Questionnaire and laboratory results will be combined
and presented for evaluation by the medical staff. You will then receive a call
from a ProactiveHealth doctor or nurse practitioner who will discuss the details
of your results and whether a natural hormone prescription is needed.
How much are the natural hormones? Are they covered by insurance?
They generally run $1 to $2 per
day. Some prescription drug plans now cover natural hormones, so you may pay only
a co-pay if you have such coverage. ProactiveHealth will check your plan to see if you have
coverage before any prescription is provided. Medicare does not now have prescription drug
coverage.
How do I know if I need natural
hormone optimization?
If your hormones are not
in balance or not in the normal range, and you are having symptoms, the medical provider will recommend a customized natural
hormone prescription. All ProactiveHealth natural hormone prescriptions are plant
derived and are chemically identical in structure to the hormones produced by human body. Because each prescription is
individually made, it can be adjusted until your symptoms are effectively addressed.
If you do need prescription natural hormones, they will be mailed to you with instructions on
use. Your ProactiveHealth provider will discuss your options, including cream, gel or capsule
form. Most prescription drug plans cover natural hormones. Before we ship any prescription,
we will call to see if you have prescription coverage and arrange for your co-pay.
How do I get started today?
If you have health insurance,
you pay your co-pay or $249 if you have no insurance, plus $5 shipping. You can pay with a credit card today,
and a kit will be mailed to you. We also need your health insurance information so ProactiveHealth
can submit the testing and consultation to your health insurance company. A form for your
insurance information and to provide us with your authorization to bill your carrier is
included in your test kit package.
What is ProactiveHealth? Are
there doctors on staff?
ProactiveHealth is a health services
company with a medical clinic and pharmacy located in the Phoenix, Arizona area. The medical
staff includes an MD, naturopathic physician and nurse practitioner.
If I need a hormone prescription,
how do I get it?
After discussing your test results,
the ProactiveHealth doctor or nurse practitioner can write a custom prescription for you. The pharmacy
can fill the prescription and mail it directly to you. You will be contacted before the prescription
is filled to arrange for payment and to get your prescription drug coverage information.
How long does it take from ordering to
receiving my hormones?
Within two weeks of submitting the saliva
sample to the lab, you should have completed a discussion of your test results with a ProactiveHealth
medical provider and have your first prescription.
Are the hormones customized?
Yes, each prescription is individually
handmade. The formula is based on your symptoms and test results and can be adjusted until all your
symptoms disappear.
How do I know these are safe?
Natural are made
from plant sources (soy and wild yams) and are identical in chemical structure to those produced by
the human body. Unlike synthetic hormones they are easily metabolized by the human body.

Many studies have been done on the use of

Where can I go locally to get it done?
The testing is done with saliva in your home. There is no need to go to a lab.
If I am currently taking a
synthetic hormone (for example, Premarin, Prempro or Provera), can I be tested without
stopping my hormones?
Yes, you don't need to stop taking the
synthetic hormones. ProactiveHealth knows the formulas that will convert your synthetic prescription
to a natural hormone formula. By testing you can see how well your current formula is working and also
look at other hormones like testosterone and DHEA.
If I have had a hysterectomy, do I still need hormones?
Yes, you probably need natural hormones.
Your own hormones are needed to protect you against osteoporosis, heart disease, breast, uterine, and ovarian
cancer. The body's natural hormones have a purpose and cannot be neglected. Testing will determine your need.
If I am still having periods, when should I do the test?
You will do the saliva test on the 20th to
the 23rd day of your cycle. This is important for measuring the normal range as a woman's hormone levels
fluctuate through the monthly cycle. Day one is the 1st day of your period. Put a reminder on your mirror
so that you don't miss the time of month to do the saliva test.
Do men or adolescents have hormones problems too?
Yes, men with diminished testosterone can have
low energy levels, weight gain or low sex drive. Young people suffering from acne or severe PMS could have hormone
balance issues. You can order a test for your husband or other family member.
